Thyroid Awareness Month 2026: Energy, Levothyroxine and Exercise That Fits

Thyroid Awareness Month 2026: Check TSH Before You Blame Willpower

January is Thyroid Awareness Month, marked by groups such as the American Thyroid Association. The thyroid is a small gland with a large vote on energy, temperature, heart rate, bowel rhythm and how heavy training feels. In India, hypothyroidism is common in clinic rooms, especially among women, and it is still under-tested in people who only present as "I cannot lose weight".

Underactive thyroid (hypothyroidism) can look like fatigue, dry skin, constipation, feeling cold, low mood, heavy periods, and a scale that does not match how little you are eating. Overactive thyroid (hyperthyroidism) can look like the opposite: heat, tremor, anxiety, loose stools, palpitations, and weight loss you did not train for. Both can wreck a gym plan. Neither is fixed by a "thyroid detox tea".

If you have a racing heart, new severe palpitations, collapsing, or swelling of the neck with breathing trouble, that is urgent care. This guide is for people with diagnosed or suspected thyroid disease who want to keep walking and lifting without fighting their own chemistry.

Get the test, then get the context

TSH is the usual starting blood test. Free T4, and sometimes T3 or antibodies (TPO), help tell the story. One slightly off number is not always a lifelong sentence. Biotin supplements can interfere with some assays, so tell the lab what you take. Timing matters if you are already on levothyroxine: many people take it on an empty stomach, then wait before coffee, calcium, iron or soy-heavy breakfasts, as their endocrinologist directed. Do not copy a neighbour's timing.

If you are planning pregnancy, thyroid needs often rise. Do not wait for a social-media "Thyroid Month hack". Call your obstetric and endocrine teams. Training in pregnancy follows obstetric rules, not gym influencer TSH charts.

How to train with hypothyroidism

Once replacement is being titrated, most people can and should move. Start with walking and two full-body strength sessions. Intensity can wait until sleep and resting heart rate look less chaotic. Cold intolerance is real; a warm-up that would bore a hyper-caffeinated 22-year-old is useful. Progress loads slowly. Muscle and tendon still adapt. They just might need an extra week at each step.

Do not use extra thyroid hormone as a fat-loss drug. That is how people get arrhythmias and bone loss. If the scale is stuck after TSH is in range, look at protein, walking, sleep, and a modest calorie gap, the same as anyone else. The gland was a bottleneck. It is not a personality.

A forest path for easy walking on days when energy is limited

Hyperthyroidism, Graves and the heart

If you are overactive, skip the "more cardio to burn it off" idea until a clinician clears you. Palpitations plus intervals is a bad mix. Walking, mobility and very moderate strength may still be allowed. Ask. Beta blockers, if prescribed, will change how hard a session feels. That is the medicine working, not you getting unfit overnight.

A week that respects energy

  • Two strength days: squat pattern, hinge, push, pull, carry. Two sets is a valid start.
  • Most other days: 20 to 40 minutes of walking, preferably outside if AQI allows.
  • One optional easy cycle or swim.
  • Sleep: protect it. Tired plus under-replaced thyroid plus 5 a.m. HIIT is how people quit.

Log energy after sessions for two weeks and show it at the next appointment. "I feel awful" is vague. "I can walk 25 minutes but stairs after squats take two days to recover" is useful.

Food, iodine and the internet

In iodine-sufficient places, drowning in kelp tablets is not a strategy. If you have autoimmune thyroid disease, some clinicians discuss selenium and other details. That is individual. Crash cabbage diets and "adrenal resets" are not. Take levothyroxine as prescribed. Do not share leftover tablets. Doses are not one-size.

After delivery, doses may change again. Fatigue is multifactorial: sleep, iron, mood, thyroid. Get labs rather than guessing.

FAQ

Can I lift heavy with hypothyroidism?

Often yes, once you are replaced and cleared, with the same form rules as anyone else. Start lighter than your ego wants.

Should I train fasted on levothyroxine?

Many people take the tablet fasted, then wait, then eat, then train later. Follow your prescriber. Do not invent a 90-minute fasted HIIT window on day one of a new dose.

Why am I still tired if TSH is "normal"?

Sleep, iron, depression, B12, overtraining and life stress are still allowed to exist. Ask for a proper work-up rather than buying a third supplement.

Does coconut oil fix thyroid?

No.

Can I do yoga only?

Yoga helps stiffness and anxiety. Pair it with walking and strength if you want bone and muscle to stay in the conversation.

Is goitre a gym stop?

Get it examined. Large goitres, nodules with worrying features, or compressive symptoms need medical care before you worry about a bench press.
